Young Leaders Award Sponsored by Apotex Nomination Form

The Young Leader Awards celebrate the efforts of up to three young leaders starting their careers in pharmacy practice. The recipients will receive an award to commemorate their contributions to the pharmacy profession and complimentary conference registration in the following year.The Young Leader Awards are open to pharmacy students (interns) in their final year of study who have made a professional contribution to patient care, the pharmacy profession or amongst their colleagues and peers at the University of Manitoba’s College of Pharmacy.

Nomination Details:

Letter of nomination;
Curriculum vitae of the nominee – limited to five pages;
Letters of support – a maximum of three additional letters of support, each describing how the nominee meets the criteria of the award for which they are being nominated;
Auxiliary documents – a maximum of two pages of additional information in support of the nomination may be provided;
These guidelines are suggested; however, all nominations submitted in any given year will be considered by the Committee.
